Our objective is to provide the opportunity for people to feel empowered and confident to manage their own support package. As part of the service at Future Links, we will support you to create your own care plan that is centred around your needs.
We offer a choice of two plans to support you on your journey to managing your package of care.
Tech Plan
The tech plan provides you access to all of our services via electronic platforms in a way that adopts our person-centred approach, yet facilitates a time effective process.
Home Plan
The home plan is designed to provide face to face opportunities where we will visit you in your own home as well as to meet with your staff in person.
Which ever plan is right for you, your chosen Home or Tech plan can be tailored with a range of our services.
These include:
